How to complete the I-9 (Employment Eligibility Verification) Form.

Within three days beginning work, the participant and employer must complete form I-9 (Employment Eligibility Verification).

  1. In Section I, the participant checks the third box and indicates dates as listed in Section III of the DS-2019 form. The host site should indicate the DS-2019 number in the Alien or Admission # field.
  2. The participant signs in the Employee's Signature section.
  3. In Section II, the employer needs to see the participant’s passport (pages listing vital statistics and J-1 visa stamp), white original copy of form DS-2019, and white form I-94.
  4. Record all three lists as follows:
    List A - Record Passport and I-94 card numbers
    List B – Record J-1 Visa
    List C – Record DS2019 Number
